Allow for 2-Part or linked Custom Questions

For custom questions such as when asking Yes/No disclosure questions or if there is corresponding supplemetary material, the follow-up question where details are listed should be linked to the initial question so that authors who answer "No" have the follow-up automatically listed as N/A. This would also prevent authors who select "Yes" from leaving out information in error.
