Ability to add our own tool tips to custom questions on all forms

Some of the custom questions we ask would benefit from having a tool tip for more information, rather than it taking up a lot of space on the reviewer report forms. The ability to add tool tips to our own custom questions would be really helpful for authors and reviewers, and make some of the forms look less daunting!
