Preferred name field

It would be really helpful if there was a preferred name field in account creation, so that authors/reviewers/editors can specify how they'd like to be addressed.

We'd also need the corresponding email tag creating to ensure that this could then be used.

    We use the "fax" field to host nicknames.  ScholarOne was able to change the name of this field for us in our system.
